We want to provide you with the very best care possible. Following is additional information that will help us in providing for your dental needs, along with Patient Registration Forms and Map & Directions to help save time at your first visit.

Patient Forms

Understanding your overall health is important to your dental care. Please be sure to provide us with a list of medications that you are taking, and inform us of any side effects that you may be experiencing. It is especially important for us to know about any allergies or adverse reactions you may have had to any medication or medical/dental procedure in the past.

Please provide us with a complete list of physicians that you are currently seeing, along with phone numbers if possible. We may need to contact your doctor if we prescribe medication or plan a procedure to be certain there are no conflicts with your medical care.

Financial Options

It is important to us that we maintain honest and open communication with our patients regarding financial arrangements for their care. Our entire team is dedicated to maintaining a helpful approach in financial matters, with utmost respect and consideration for your privacy and specific needs.

We ask that payment for treatment be made at the time of service. We work with each patient on an individual basis during the treatment planning process to phase treatment and schedule appointments with their budget in mind.

We accept cash, personal checks, MasterCard, and Visa. Low interest third party financing is also available through CareCredit®. *subject to approval terms of third party financing provider


Dental Insurance

We are in-network dental insurance providers for MetLife, Ameritas and Humana. We will be happy to assist you with filing your dental insurance, and ask that you pay your co-pay or amounts not reimbursed by insurance at the time of service. Please be sure to bring your dental insurance card to your first appointment.

Office Visits

We normally call you to remind you of your appointment one to two days in advance, and kindly request that you call us back to confirm your visit. Please plan to arrive 10 minutes before your scheduled appointment time so that we may update your records or review any pertinent information with you prior to being seen.

We do not double-book in our office, and sincerely appreciate your support in making sure that appointment times are used effectively to care for patients in need of treatment.

In order to provide timely access to care to all of our Valued Patients, we request that you contact us at least 24 hours in advance if you must cancel or reschedule your appointment.


We want your first visit to our office to be as comfortable as possible. Please do not hesitate to Contact Us to assist with any special arrangements that may be required for your care.
